Once you have decided to partner with Yes to Home Care as your home care provider, you will undergo an initial living health assessment to diagnose your needs. We will work hand in hand with your personal physician for the review of your medical history and need, after which our providers will draft an individualized service plan that is customized to meet your specific needs in the home setting.

WHAT IS PCS?
- Personal Care Services is a Medicaid benefit that assists eligible clients who require assistance with activities of daily living and instrumental activities of daily living because of a physical, cognitive or behavioral limitation related to their disability, physical or mental illness, or chronic condition.
The following needs of the parent/Guardian are also considered:
- The following needs of the parent/guardian’s need to sleep, work, attend school, meet his/her own medical needs.
- The Parent /guardian’s legal obligation to care for, support , and meet the medical, education, and psychosocial needs of his/her other dependents.
- The parent/ Guardian’s physical ability to perform the personal care services.
PCS does not include:
- Services when the client has the physical, behavioral, and cognitive abilities to perform an ADL and/or IADL without adult supervision.
- Respite care or child care.
- Protective Supervision.
We at Yes to Home Care, provides professional caregivers to assist your loved to provide Personal Care Services. PCS services are for anyone under the age of 21 who is enrolled in Medicaid and is in need of some assistance for their activities of daily living.
